SELF DIRECTED HEALTH TESTING
Blood and Urine testing -- Frequently Asked Questions

Q:Where do I go to get my blood drawn?
A: For almost all the test panels, you will go to a Quest Laboratory draw station, where your blood will be drawn at no extra charge to you. We can help you locate one nearest your zip code. For people living in and around Harrison, Arkansas, we can draw the blood at our office, Harrison Optimal Health, 6302 Hillside Lane, Harrison AR. The exception to this is the food antibody (IgG) testing, which needs to be drawn at our office, or at your doctor's office where they will be able to prepare the specimen for shipment to Great Plains Lab. You will be mailed a kit with complete instructions for the urine testing.

Q: Which laboratory will be performing the tests on my blood specimen?
A: Most of the laboratory panels will be done at Quest Laboratories. The food antibody profile testing is done at Great Plains Laboratory in Lenexa, Kansas, where they are a leader in nutritionally related lab testing.

Q: Can I order laboratory testing if I live outside of Arkansas?
A: The answer for many of you will be yes. However, if you live in California, Oklahoma, New Jersey, New York, Massachusetts or Rhode Island, we are unable to order testing for you, due to state law or due to lack of computerized access to Quest Labs in these states.

Q:Who actually orders the blood testing for me?
A: Our staff physicians at Harrison Optimal Health will order the blood testing.

Q:Will I receive a prescription for the blood testing?
A:You will receive a Quest "Lab Order Confirmation" in the mail, USPS first class, which you will take with you for the blood draw.

Q:Is there an expiration date on my blood test order?
A: YES. Your requisition(Lab Order Confirmation) is only good for 6 months after the date your order is placed. There will be no refunds issued if the blood draw is not done in this amount of time.

Q:How and when will I get my blood test results?
A:You will receive your results in the mail (USPS first class) within two weeks of the date of your blood draw, along with educational materials about the interpretation of the test. Many tests be be available within just a few days. The only test that takes longer than 2 weeks is the IgG Food Antibody Profile, which usually takes 3 weeks or slightly longer for results.

Q: What is the advantage of ordering testing through Future Visions Foundation?
A: You will not have a doctor’s office visit to pay for to order tests through Future Visions Foundation. In partnership with Quest Labs, we can offer you deep discounts compared to what you would spend most other places. In addition, some doctors are not willing to order tests for prevention and wellness purposes, due to insurance company contingencies. Testing performed by Quest Laboratories will be respected by your doctor, and provide useful and needed information to your health care practitioner.

Q:Will insurance cover any of these blood tests?
A: No insurance coverage is available, because we cannot provide you with the codes (ICDM-9,CPT codes) that are required for insurance payment. When you order a test from Future Visions Foundation, you do so with the understanding that you are paying privately for these tests. For insurance coverage, we would have to do a doctor's appointment and evaluation before ordering, and charge higher prices for the entire process. There is absolutely no billing to Medicare, Medicaid, or any private insurance. That's how we keep the cost of testing low.

Q: Will there be a fee at the draw station?
A: If you have the IgG food antibody profile drawn by your local doctor's office, there may be a draw fee charged to you. The draw fee for any Quest draw station is included in the price of your blood testing, so there will be no extra fees charged for the other panels.

Q: Can I order the add-on tests by themselves without ordering one of the panels of testing?
A: The add-on tests are intended to be added onto the Major Blood Testing Panels (General Health and Wellness Panel, the Metabolic Syndrome Panel, the Male or Female Hormone Panel, or the Cardiovascular Risk Panel). However, you may order any of the add-on tests or combination of the add-on tests separately without purchasing any of the panels listed above. You will be charged a $10.00 additional charge to cover the blood draw fee. You will not be charged any additional draw fee at the blood draw station. This $10.00 fee will be added to the total purchase price of any add-on or combination of multiple add-on tests.
